Key Responsibilities
  • Support primary care objectives through targeted outreach to patients, data mining, and surveillance of health maintenance areas.
  • Coordinate care with the broader care team, including Pharmacists, Care Managers, and Social Work.
  • Monitor work queues and proactively create relationships with providers and patients to ensure optimum health and wellness for our patient population.
  • Collaborate with healthcare teams to provide individualized, innovative, and specialized care to patients.
Requirements
  • High School Diploma or equivalent required.
  • Must meet current eligibility to sit for the exam with attainment of certification or registration through one of the Penn State Health approved agencies for Medical Assistants within 90-days of employment date or current active status as a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) through one of the Penn State Health approved agencies for Medical Assistant certification required.
