Pharmacist - Optimal Pharmaceutical Care Specialist

As a Pharmacist at Catholic Medical Center, you will play a vital role in delivering high-quality pharmaceutical care to patients. Your expertise in assessing, responding, and adapting treatments will enable you to make a meaningful impact in a dynamic healthcare environment.
Key Responsibilities
- Review and verify provider medication orders to ensure accuracy, interactions, compatibility, and appropriateness of therapy.
- Provide updated drug information to hospital staff and patients, and participate in the management of distribution, inventory, purchasing, and accountability of controlled substances.
- Compound and/or dispense medications, intravenous solutions, and chemotherapy medications in compliance with state and federal rules and laws.
- Monitor drug therapy for contraindications, incompatibilities, allergies, dosing errors, adverse reactions, and overall appropriateness of therapy.
- Review patients' home medication lists for accuracy and appropriateness, intervening when necessary.
What We're Looking For
- A degree in Pharmacy from an institution accredited by the American Council on Pharmaceutical Education.
- One (1) year of previous pharmacy experience is preferred.
- A New Hampshire license to practice Pharmacy.
- The ability to establish and maintain positive working relationships within the department and with other hospital departments, patients, employees, regulatory agencies, vendors, and the public.
- The ability to plan and perform a wide variety of duties requiring general knowledge of policies and procedures applicable for that area.
