Job Summary

We are seeking a highly skilled Physical Therapist to join our team at Complete Home Care. As a Physical Therapist, you will play a vital role in providing coordinated care to clients of all age groups, restoring or maintaining patient well-being through the implementation and evaluation of client care plans.

Key Responsibilities
  • Plan, implement, and evaluate client care plans to restore or maintain patient well-being.
  • Organize and conduct physical therapy treatment based on medical referrals and evaluations.
  • Provide effective communication to clients, their families, team members, and other healthcare professionals.
  • Monitor assigned cases to ensure compliance with requirements of third-party payers.
  • Complete appropriate documentation in a timely manner to assure compliance with company policy.
Requirements
  • Two (2) years of experience as a PT in a clinical care setting.
  • Home health experience.
  • Therapy skills as defined by generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision-making skills.
  • Current driver's license in good standing.
  • Reliable transportation and auto liability insurance.
  • Current State professional license as a PT in any/all applicable states.
  • Current CPR certification.
Physical Requirements
  • Prolonged standing/walking required.
  • The ability to lift/transfer up to 50 lbs.
  • Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions.
Environmental/Working Conditions
  • Works in patient's home in various conditions.
  • Possible exposure to blood-borne pathogens, bodily fluids, and infectious diseases.
  • Some exposure to unpleasant weather; PRN emergency call.
