Turning Point Community Programs is seeking a dedicated professional to fill the role of Intake Coordinator at our Sage Village program in Stockton. As a key member of our team, you will play a vital role in providing psychosocial, rehabilitation, and recovery-oriented services to our members.
Key Responsibilities- Develop and maintain positive working relationships with referring hospitals and outpatient providers.
- Coordinate intake functions with the Crisis Residential Programs, Intake Team, and all referral sources.
- Conduct interviews in the community and make recommendations for admission to the program.
- Complete intake paperwork with clients upon admission to the program.
- Ensure that necessary documentation is received and processed from referral sources prior to client admission, including medication orders and medication.
- Introduce clients to the staff on-duty, the assigned service coordinator, and other clients in the facility, provide a tour of the facility, and show the client to a clean room.
- Confer with program leadership if the client has specific needs and risk factors, for example, medical issues, dietary needs, and language services.
- Participate in in-service training and workshops.
- Assess clients for involuntary hospitalization per WI 5150 and complete necessary forms.
- Provide prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
- Provide on-the-spot counseling that is both helpful to the clients and consistent with the philosophy of the program.
- Conduct psychosocial groups, treatment planning, and discharge planning activities.
- Ensure the safety, health, and well-being of the members.
- Complete paperwork as assigned in a timely, thorough, and efficient manner.
- Provide support to other staff members as needed.
- Attend community meetings to represent the Program as assigned.
- A master's degree in Social Work or a related field.
- Valid registration with the Board of Behavioral Sciences (BBS).
- A California driver's license and current vehicle insurance/registration if driving.
- Reliable means of transportation capable of passing a vehicle safety inspection if more than five years old.
This is a full-time position working Monday through Friday, 8:00 AM to 4:30 PM. We offer a competitive compensation package and a supportive work environment.
