About the Role

We are seeking a highly skilled Community Care Specialist to join our team in Monroe County, NY. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary

The Community Care Specialist will work with our interdisciplinary team to provide comprehensive care to patients in their homes, nursing homes, and assisted living facilities. This is an exciting opportunity to work in a fast-paced environment where no two days are ever the same.

Key Responsibilities
  • Conduct comprehensive assessments of patients' medical needs
  • Develop and implement personalized care plans to address patients' complex behavioral, social, and functional needs
  • Manage patients through acute medical situations or exacerbations of chronic conditions
  • Ensure accurate and complete documentation of patient interactions and care plans
  • Collaborate with the interdisciplinary care team to coordinate patient care
Requirements
  • Education: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (NP) or Physician Assistant degree program (PA)
  • Active and unrestricted license in the state of New York
  • Current, active DEA licensure/prescriptive authority
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and varying dwelling conditions
Preferred Qualifications
  • 1+ years of clinical experience in practice, preferably in long-term care settings
  • Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs
