Palliative Care Nurse Practitioner

Summit Recruiting Group is seeking an experienced and compassionate Palliative Care Nurse Practitioner to join our team in Astoria, Oregon.

We offer a competitive salary of $120,000 per year and a comprehensive benefits package.

The ideal candidate will have a Master of Science in Nursing or Doctor of Nursing Practice from an accredited program, with experience in palliative care and hospice desired. Certification as a Geriatric, Family, or Adult Nurse Practitioner licensed in the State of Oregon is required. Advanced Certification in Hospice and Palliative Nursing (ACHPN) is preferred.

Job Description
As a Palliative Care Nurse Practitioner, you will work collaboratively with patients, their families, and other caregivers to deliver high-quality medical management of symptoms related to advanced illness. You will be responsible for coordinating care with attending physicians, arranging support services, and recognizing and managing unusual changes in condition.

Key Responsibilities:
  • Coordinate care with attending physicians, eliciting pertinent history details and disease status.
  • Arrange support services as needed by collaborating with the Palliative Care Social Worker.
  • Contact attending physician to discuss emergent situations and plans as appropriate.
  • Recognize and manage unusual or unexpected changes in condition and contact attending physician as appropriate.
  • Discuss risks/benefits when prescribing medications or treatments.
  • Respect patient diversity in beliefs and healthcare concerns.
  • Participate in ongoing education in hospice and palliative care.
  • Record all patient visits, including lab results, medication changes, current plan of care, return visits, and new problems in the patient's electronic health record (EHR).

Requirements:
  • Certification as a Geriatric, Family, or Adult Nurse Practitioner licensed in the State of Oregon.
  • State of Oregon Prescriptive privileges required.
  • ACLS/BLS through American Heart Association required.
  • Advanced Certification in Hospice and Palliative Nursing (ACHPN) is preferred.

Education and Experience:
  • Master of Science in Nursing or Doctor of Nursing Practice from an accredited program.
  • Experience with palliative care is strongly preferred.
  • Experience with hospice desired.
