About the Role:

As a Hospice RN Case Manager at SouthernCare, you will play a vital role in providing compassionate care to patients and their families. Your primary responsibility will be to report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.

Key Responsibilities:

Act as the primary liaison between the physician, patient/family, and hospice team.
Provide direct patient care services to hospice patients and supportive care to the patients' family with a reasonable case load while providing a trusting and compassionate relationship with your patients and their families.
Empower patients' caregivers by educating them on care for their loved one.
Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
Maintain all assigned patient clinical records.
Participate in company quality assessment and performance improvement programs.

About You:

We are looking for a compassionate and empathetic individual who is comfortable providing care wherever the patient calls home. You should demonstrate flexibility and composure while supporting others in a time of crisis. A RN license in the state of requested employment (or eligibility to obtain) is required, along with 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology. Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing. Current drivers license and automobile insurance are also required. Current CPR certification is a plus.
