LHC Group is seeking a skilled Registered Nurse (RN) to join our Home Health team in Melbourne, Florida.
About the Role:We are looking for a compassionate and dedicated RN to provide high-quality patient care in the comfort of their own homes. As a Registered Nurse with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Conduct comprehensive assessments of patients to identify their healthcare needs and develop individualized care plans.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Current RN licensure in the state of Florida.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
