**Job Summary:**

We are seeking a highly skilled and compassionate RN to join our team as a Medical Care Coordinator. In this role, you will be responsible for linking mobile resources together, integrating and coordinating them to respond appropriately to patient needs.

The ideal candidate will have 5 years or more of RN experience in Emergency or Acute Care settings and a strong background in clinical data gathering and Ethical Principles. A Compact RN License and current CPR card are also required.

In addition to your medical expertise, you will possess excellent communication skills, both verbal and written, and the ability to work effectively with patients, caregivers, referral sources, physicians, and staff members.

As a Medical Care Coordinator, you will telephonically triage patients, navigate care based on evidence-based protocols, and provide patient education across the healthcare continuum. You will also engage healthy patient behaviors through telemonitoring activities and participate in training courses to promote the AMR Integrated Healthcare Program.

Our company offers a competitive salary range of $38-$42.50 per hour, with additional shift differentials available for Weeknights and Weekend shifts. We also offer a retention bonus of $4,000 and a comprehensive benefits package, including medical, vision, dental, 401k, disability, FSA, HAS, EAP, vacation, and paid time off.
