Position Summary

We are seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team of skilled home health care providers. The LPN will provide skilled nursing care under the direct supervision of a Registered Nurse (RN). They will perform selected skilled actions in the delivery of comprehensive patient care, ensuring the well-being and comfort of our clients.

Responsibilities

  • Provide direct patient care, including wound care, medication administration, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with the RN and other healthcare professionals.
  • Educate patients and families on health conditions, medications, and self-care techniques.
  • Document patient assessments, interventions, and outcomes accurately and thoroughly.
  • Maintain a safe and clean environment for patients and staff.

Qualifications

  • Current, unencumbered LPN license in the state of Arizona.
  • Experience providing home health care is preferred.
  • Strong clinical skills and knowledge of nursing principles.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
