Client Details and Schedule:
The client is situated in a designated area with a work schedule from Monday to Friday, 8 AM to 4 PM.
Key Responsibilities:
Health Conditions: Post-stroke recovery, Diabetes management, Arthritis in ankles, Vision impairment in the left eye, Neuropathy in feet.
Goals & Objectives: Assist with all transfers using appropriate safety equipment. Encourage the client to utilize the living room chair frequently. Promote independence while ensuring safety. Provide assistance with showering on designated days, including help with dressing and personal hygiene. Support with denture care if necessary, and prepare nutritious meals considering dietary restrictions. Ensure hydration, perform light housekeeping tasks, and engage in meaningful conversation throughout the shift. Report any observed changes in condition. May involve transportation to medical appointments or errands.
Activities of Daily Living (ADLs):
Mobility: The client can transition from lying down to sitting independently. Requires assistance for walking, especially with a gait belt for safety. Needs help entering and exiting vehicles, with additional support on the weaker side.
Toileting: The client does not experience incontinence and can communicate needs effectively. Assistance to the bathroom is required while respecting privacy.
Bathing/Dressing/Grooming: The client prefers showers every other day and requires help with entering and exiting the shower, as well as drying off and dressing. Assistance with hair care is needed weekly, while the client can select clothing and brush teeth independently.
Nutrition: Meals need to be prepared or warmed, and the client can feed herself. Reminders for hydration and healthy eating habits are essential.
Communication: The client communicates clearly, with no cognitive impairments, and enjoys discussing personal interests.
Qualifications:
- Minimum of one year of experience as a caregiver, CNA, or similar role
- Strong verbal and written communication skills
- Valid driver's license and appropriate auto insurance
- Willingness to undergo background checks
- Consent to drug and alcohol testing
- TB test required
