Nurse Care Coordinator

Job Description:
We are seeking a Nurse Care Coordinator to join our team at Northern Light Inland Hospital's OB-GYN Unit. The ideal candidate will be a registered nurse with excellent communication and organizational skills, able to coordinate patient care and ensure seamless transitions between departments.
About the Role:
This role involves working closely with interdisciplinary teams to develop and implement individualized care plans for patients. As a Nurse Care Coordinator, you will also be responsible for communicating effectively with patients, families, and healthcare providers to ensure continuity of care.
Salary and Benefits:
Our estimated annual salary for this position is $72,000 to $90,000 based on experience, along with a competitive benefits package that includes medical, dental, and vision coverage, retirement savings, and paid time off.
Key Responsibilities:
- Coordinate patient care across departments and services.
- Communicate effectively with patients, families, and healthcare providers.
- Develop and implement individualized care plans.
- Monitor and evaluate the effectiveness of care plans.
- Maintain accurate records and reports.
Required Skills and Qualifications:
- Registered Nurse license required.
- High school diploma or equivalent required.
- BLS certification required.
