Job Title: Registered Nurse 2

Job Summary:

We are seeking a highly skilled and dedicated Registered Nurse 2 to join our team. The successful candidate will provide professional nursing services to individuals in the Family Care Unit, ensuring their health and well-being are met.

Key Responsibilities:
  • Provide health care oversight to individuals receiving Family Care services.
  • Coordinate and follow up on each individual's health care in partnership with the Family Care Provider.
  • Complete nursing assessments and develop and monitor plans of care to address individuals' health care needs.
  • Effectively communicate and coordinate with other healthcare providers to ensure healthcare recommendations are carried out.
  • Train Family Care Providers to ensure they follow any healthcare needs of the individuals in their care.
Requirements:
  • Must maintain a valid New York State license and registration.
  • Currently a permanent Registered Nurse 2 (Community) or eligible to transfer via Section 70.1 of the Civil Service Law.
  • Two years of post-licensure clinical nursing experience, with one year providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
