Key Responsibilities:

  1. Client Evaluation and Coordination:
    • Perform initial evaluations of elderly clients to ascertain their care requirements and preferences.
    • Organize and schedule caregiver visits according to client needs, ensuring optimal caregiver-client matching.
    • Keep accurate and updated client records, including care plans, preferences, and any modifications in health status or care needs.
    • Engage in regular communication with clients and their families to address inquiries, provide updates, and ensure satisfaction.
  2. Caregiver Management and Support:
    • Recruit, assess, and interview potential caregivers to create a competent and dependable care team.
    • Deliver orientation and training to new caregivers, ensuring they comprehend client care plans and expectations.
    • Assign caregivers to client cases based on their availability, expertise, and compatibility.
    • Conduct routine check-ins with caregivers to address issues, offer support, and ensure compliance with care plans.
    • Promote a positive work environment by acknowledging caregiver achievements and addressing performance challenges.
  3. Care Plan Development and Execution:
    • Collaborate with clients, their families, and healthcare professionals to create tailored care plans.
    • Ensure caregivers are well-informed about client care plans, including medication schedules, dietary restrictions, and mobility assistance.
    • Regularly review and adjust care plans based on client feedback, evolving needs, or recommendations from healthcare professionals.
    • Coordinate with caregivers and other healthcare providers to ensure seamless execution of care plans and effective communication.
  4. Documentation and Reporting:
    • Maintain precise and current documentation of client information, caregiver assignments, and care-related activities.
    • Prepare reports and summaries detailing client progress, caregiver performance, and any incidents or concerns.
    • Provide necessary documentation and reports to supervisors, healthcare professionals, and regulatory bodies as required.
  5. Quality Assurance and Compliance:
    • Comply with all relevant laws, regulations, and industry standards in senior home care services.
    • Implement quality assurance measures to monitor and evaluate the effectiveness of care services.
    • Conduct periodic client satisfaction surveys to identify areas for improvement and implement necessary changes.
    • Collaborate with the management team to develop and implement policies, procedures, and training programs to enhance service quality.

Qualifications and Requirements:

  • Degree in a healthcare-related field or experience as a retired nurse (additional education in healthcare or social services is advantageous).
  • Preferred experience in homecare.
  • Demonstrated experience in coordinating care services, ideally in senior home care or a related sector.
  • Strong organizational and multitasking abilities to manage multiple client cases simultaneously.
  • Excellent communication and interpersonal skills to effectively engage with clients, caregivers, and other stakeholders.
  • Proficiency in using computer systems and software for documentation, scheduling, and reporting.
  • Understanding of senior care needs, common health conditions, and care plan development.
  • Ability to work independently, make informed decisions, and solve problems effectively.
  • Compassion, empathy, and patience in interactions with elderly clients and their families.
  • Familiarity with local regulations and compliance requirements in senior home care services.
