Position Overview:
We are currently seeking a skilled RN Field Nurse/Case Manager to enhance our hospice program. In this role, you will have the unique opportunity to provide personalized care to patients, working collaboratively within an interdisciplinary team under the guidance of the Medical Director and/or attending physician. Your responsibilities will include:
- Conducting skilled nursing assessments
- Planning and coordinating services to ensure the comfort and well-being of patients and their families
Qualifications:
To be considered for this position, candidates should possess:
- A degree from a nursing school accredited by the National League for Nursing; a Bachelor's degree in nursing is preferred
- Current registration, certification, or licensure as mandated by state regulations
- A valid driver's license and auto insurance, along with reliable transportation during work hours
- Experience in hospice or home health care, particularly with terminally ill patients, is advantageous

Employment Types:

Compensation:
Annual salary range of $70,000 to $80,000
