Skilled Practical Nurse

Position Overview
As a Licensed Practical Nurse, you will play a crucial role in coordinating internal referrals and managing the intake process for patients transitioning from various healthcare facilities. You will serve as a vital link between hospitals, medical offices, home health agencies, and medical suppliers.
Key Responsibilities
- Patient Assessment: Gather and submit patient information for evaluation to determine the suitability of homecare services. Ensure all necessary documentation is completed in accordance with agency and hospital policies. 10%
- Referral Management: Keep homecare staff updated on potential referrals from hospitals. Efficiently process patient referrals and guide them through the home care service line. Maintain clear communication with families, patients, hospital case management, physicians, and other care team members to ensure a smooth referral and intake experience. 10%
- Service Initiation: Initiate necessary referrals and specialty services. Clearly communicate home health, hospice, and monitored at-home services to patients and their families. Explain admission criteria for homecare services. 10%
- Intake Coordination: Accurately complete the intake process to facilitate seamless transitions from hospital to home care services. 10%
- Continuity of Care: Maintain ongoing communication with hospital case management and other clinical staff to ensure continuity of care. Act as an educational resource for team members and liaise with physicians, hospitals, and medical equipment companies. 10%
- Team Collaboration: Actively participate as a liaison with physician offices and medical support services. Assist in training and community education programs. Adhere to policies that promote safe working conditions and report any inappropriate patient care referrals. 10%
- Home Visits: Conduct routine and telehealth visits in home care settings. Support medication management and facilitate patient education regarding medication delivery and administration. Schedule necessary clinician visits and ensure compliance with regulatory requirements. 10%
- Quality Assurance: Perform quality audits as required. 10%
- Support Roles: Provide assistance to assistant managers, RNs, CNAs, and office personnel. 10%
- Medication Administration: Assist with medication administration and patient rounds in various facilities. 10%
This position does not include management responsibilities and will report to a supervisor or manager.
Educational Requirements
Must be a graduate of an accredited nursing program.
Experience Requirements
Minimum of 1 year of experience in home care settings.
Licensure Requirements
Must hold a valid LPN or RN license in South Carolina and possess a South Carolina driver's license.
Work Schedule
Weekend shifts available.
