Position Overview
The role involves coordinating internal referrals and managing the intake process for patients transitioning from various healthcare facilities. This position serves as a vital link between hospitals, medical offices, home health agencies, and medical suppliers.

Key Responsibilities
  • Patient Assessment: Gather and submit patient information for evaluation regarding the suitability of homecare services. Ensure all necessary documentation is completed in accordance with agency and hospital policies. 10%
  • Referral Management: Keep homecare staff updated on potential referrals from hospitals. Accurately process patient referrals and oversee them through the appropriate home care service channels. Maintain clear communication with families, patients, hospital case management, physicians, and other care team members to ensure a smooth referral and intake experience. 10%
  • Service Coordination: Initiate relevant referrals and specialty services. Effectively communicate changes and present home health, hospice, and monitored services to families and patients. Clearly explain homecare services and admission criteria to patients and their caregivers. 10%
  • Intake Processing: Ensure accurate and efficient intake procedures to facilitate seamless transitions from facilities to home care services. 10%
  • Continuity of Care: Maintain ongoing communication with hospital case management and other services to support continuity of care. Collaborate with all involved staff, including physicians and therapists, to enhance patient care. Act as an educational resource for team members. 10%
  • Liaison Activities: Actively participate as a liaison with physician offices and medical support services. Assist in training and community education initiatives. Uphold compliance with safety policies and recognize inappropriate patient care referrals. 10%
  • Telehealth Support: Conduct routine and telehealth visits as part of home care services. Support medication management and provide education to patients regarding medication delivery and usage. Facilitate appropriate communication and scheduling of clinician visits. 10%
  • Quality Assurance: Complete quality audits as requested. 10%
  • Team Support: Provide assistance to managers, RNs, CNAs, and office personnel. 10%
  • Medication Administration: Assist with medication administration and patient rounds at designated facilities. 10%
Supervisory Role
This position does not involve management responsibilities and reports to a supervisor or manager.

Educational Requirements
Must be a graduate of an accredited nursing program.

Experience Requirements
Minimum of one year of experience in home care settings.

Licensure and Certifications
Must hold a valid LPN or RN license in South Carolina, along with a South Carolina driver’s license.

Work Schedule
Weekend shifts available.
