Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at ChenMed. As a key member of our clinical operations team, you will be responsible for providing direct patient care to our geriatric patients, working collaboratively with our physicians and other healthcare professionals to ensure the highest level of care.
Key Responsibilities- Assess and manage acute and non-acute clinical problems, performing physical assessments and documenting patient histories, analyzing trends in patient conditions, and developing and implementing patient management plans.
- Provide patient education and counseling, advocating for patients and their families, and collaborating with multidisciplinary teams to ensure seamless care transitions.
- Prescribe medications, order laboratory and diagnostic studies, and refer patients for consultation as needed.
- Participate in patient care rounds and conferences, communicating patient management strategies to team members and collaborating to ensure successful patient outcomes.
- ARNP or similar advanced degree in Nursing required.
- Current Nurse Practitioner Certification in the State of practice required.
- Board certification by AANP or ANCC is preferred but may be required for certain States.
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates.
