Job Title: Licensed Practical Nurse LPN

We are seeking a skilled and compassionate Licensed Practical Nurse LPN to join our team at The Bill and Betsy Scheben Care Center. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and support to individuals with medical, physical, social, and/or cognitive disabilities.

Job Summary:

The Licensed Practical Nurse LPN will be responsible for assisting with patient care in our Adult Day Health Center. This includes providing medical, psychological, social, and personal care to patients, as well as performing routine assessments and reporting changes to RNs, physicians, and families. The successful candidate will also act as a liaison among physicians, case managers, and families, and will be responsible for obtaining required documentation for individual charts.

Key Responsibilities:
  1. Assess patients' needs and identify potential problems.
  2. Assess adjustment to the Adult Day Health program and implement changes as needed.
  3. Provide a safe, healthy, and supportive environment, reporting any issues to maintenance and the safety director.
  4. Act as a liaison between patients and their physicians, communicating with families and outside agencies on a regular basis.
  5. Use MWMA daily to obtain required waiver documentation and track dates for care plans and annual renewals.
  6. Review and update MARs on a scheduled basis, and as needed.
  7. Maintain an appropriate ratio of staff to patients according to state regulations and patient acuity.
  8. Ensure weights and vital signs are recorded per month and as needed.
  9. Maintain a working relationship with the RN Program Manager and assist with appointments for assessments of new individuals as needed.
  10. Report all new admissions and discharges, and maintain an updated emergency sheet on all patients.
  11. Assess staff skill and identify staff weaknesses and strengths, communicating the need for additional training to the Program Nurse.
  12. Mentor and lead staff to perform to their highest ability, providing direction and implementation of treatment to meet therapeutic and rehabilitative goals.
  13. Assist with coverage in Program Rooms as needed, including assisting with feeding, hygiene, and activities.
