Position Overview
This role entails overseeing the medication management for designated patients, ensuring the evaluation and monitoring of medication therapy aligns with the patient's health condition, institutional standards, and cost-effective practices. Responsibilities include conducting medication reconciliation, monitoring therapeutic outcomes, and providing patient education. Additionally, the position involves engaging in the training of Pharmacy and Medical Residents and offering educational support to healthcare professionals as necessary. The pharmacist will also contribute expert pharmacy knowledge for clinical order sets, benefit designs, formularies, and other pharmacy-related documentation as required.

Educational Requirements
• Required: Bachelor of Science in Pharmacy
• Completion of a PGY1 Pharmacy Practice Residency (or 3 years of relevant specialty experience in lieu of PGY1 Residency)
• Preferred: Doctor of Pharmacy (PharmD) and completion of a PGY2 Specialty Pharmacy Residency

Work Experience
• Required: None

Licensure
• Required: Current Pharmacy license in the state of practice

Knowledge, Skills, and Abilities
• Proficient computer skills for data entry and patient information retrieval; familiarity with Windows-style applications and keyboard use.
• Strong verbal and written communication skills, capable of presenting information clearly and professionally to diverse audiences.
• Excellent leadership and teaching capabilities.

Key Duties

  • Provides pharmacokinetic consultations, utilizing appropriate laboratory data to assess drug selection and dosing, and communicates findings to healthcare practitioners.
  • Evaluates patient responses to medication therapy.
  • Documents interventions and conducts outcome studies to demonstrate effective patient outcomes.
  • Assists in developing clinical pathways, best practices, and guidelines for specific patient populations.
  • Educates staff, students, patients, and other healthcare professionals, with a commitment to publish findings biannually.
  • Mentors residents, students, and new staff to achieve departmental objectives.
  • Ensures comprehensive patient care throughout all transitions of care.
  • Volunteers for additional tasks during staffing shortages due to unforeseen circumstances.
  • Adapts communication and service explanations to meet the specific needs of diverse patient populations.
  • Performs other related duties as assigned.
