Job Summary

As an Inpatient Case Manager, you will be responsible for providing comprehensive care management services to patients across the healthcare continuum. Your primary focus will be on patient transition to the appropriate level of care while ensuring clinical appropriateness and length of stay. You will directly support patients identified in need of comprehensive case management, promoting effective utilization and monitoring of health services. You will collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions. You will develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver.

Responsibilities

* Assess the social, psychological, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient.
* Collaborate with patients, families, healthcare team members, and community organizations, and use strong advocacy skills to identify needs and resources in the hospital and community.
* Develop and evaluate effective transition plans taking into consideration the client's diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care.

Requirements

* 18 months of recent inpatient RN Case Management experience.
* BLS within 90 days of hire.
* BSN is preferred. All Registered Nurses who do not have their BSN will be required to sign a BSN agreement committing to successfully obtain their BSN within 5 years of hire.
* Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
