Job Summary

We are seeking a skilled Registered Nurse to join our team at IntelyCare. As a Registered Nurse, you will provide high-quality patient care in post-acute facilities, working closely with physicians and other medical staff to develop and implement effective care plans.

Responsibilities
  • Provide quality care to patients in post-acute facilities
  • Collaborate with physicians and medical staff to develop and implement care plans
  • Develop and implement best practices for patient care
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and adjust care plans as needed
  • Supervise and train nursing and other staff members
  • Document patient histories and interventions
  • Participate in incident investigations and quality improvement initiatives
Requirements
  • Current New Hampshire state license to practice as a Registered Nurse (RN)
  • Successful completion of an approved nursing program
  • At least 1 year of experience as a Registered Nurse or in a related role
  • Advanced organizational and communication skills
