Responsibilities:

  • Provide skilled nursing care to patients in their homes, focusing on their unique needs and goals.
  • Conduct thorough assessments, develop care plans, and implement interventions to promote patient health and well-being.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress, outcomes, and any changes in their condition.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions.

Requirements:

  • Minimum of 1 year of recent home health nursing experience (private duty does not qualify)
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • Current Virginia RN License
  • American Heart Association BLS
  • Valid U.S. Driver's License
