AHN Healthcare@Home is seeking compassionate and skilled Registered Nurses to join our team of dedicated home health professionals. As a Home Health Registered Nurse, you will play a vital role in delivering high-quality patient care in the comfort of patients' homes.

Key Responsibilities:

  • Lead and collaborate with interdisciplinary teams to develop and implement patient care plans
  • Provide skilled nursing care to patients in their homes, promoting independence and improving quality of life
  • Conduct thorough assessments, identify patient needs, and develop individualized care plans
  • Collaborate with patients, families, and caregivers to ensure seamless care transitions
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

  • RN license in the state of employment
  • Graduate from an approved school of practical nursing
  • One year of RN experience and clinical competence to deliver quality patient care
  • Current driver's license and ability to spend approximately 20% of the day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting and self-manage time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software

Includes compensation related to patient care visits, non-visit activities, paid days off, shift premium pay, and first year success bonus. Low end of the range is the average annual compensation earned based on a fully productive caseload. The high end of the range is reflective of those who exceed productivity.
