Intake Coordinator Opportunity at Eastern Connecticut Hematology and Oncology

We are seeking a highly skilled and detail-oriented Intake Coordinator to join our team at Eastern Connecticut Hematology and Oncology. As a key member of our clinical operations team, you will play a critical role in ensuring seamless patient referrals and access to our services.

Key Responsibilities:
  • Contact patients within 24 business hours of referral to discuss their care needs and ensure timely access to our services.
  • Review referrals for appropriateness and prioritize them based on urgency, working closely with our physicians to ensure efficient and effective care.
  • Manage electronic fax queues and patient records, ensuring accurate and timely documentation.
  • Collect and verify patient demographic, insurance, and clinical information to ensure accurate billing and care coordination.
  • Schedule patient appointments and work with our Director of Practice Operations to address any scheduling conflicts.
  • Communicate effectively with patients, referring offices, and internal stakeholders to ensure seamless care transitions.
  • Document all patient interactions and care coordination activities in accordance with HIPAA guidelines and billing requirements.
  • Collaborate with other departments and external care providers to address and resolve issues impacting patient care.
Requirements:
  • Minimum 1 year of experience in a community clinic setting.
  • Proven work experience in a role emphasizing teamwork, strategy, and community healthcare.
  • Associates Degree preferred but not required.
