About the Role:
The Children's Long-Term Support waiver is a Medicaid-funded program designed for children with severe disabilities living with their families. The clients we serve may have developmental, physical, or severe emotional challenges.
As a vital member of the Children's Long Term Support Team (CLTS), this role is focused on coordinating services for children in Milwaukee County. The Service Coordinator collaborates closely with families to establish goals and care requirements for each child, leading to the creation of a personalized service plan. This position involves identifying suitable services to achieve these objectives through waiver-funded options, insurance resources, and informal community support available to eligible families.
Key Responsibilities:
- Provide families with a robust knowledge base to assist them in making informed decisions regarding their child's options.
- Work collaboratively within an interdisciplinary team, contributing as a supportive team member.
- Exhibit strong oral and written communication abilities.
- Utilize computer-based communication tools, word processing, data entry systems, and teleconferencing platforms effectively.
- Engage in remote training and learning opportunities.
- Conduct quarterly home visits in client residences throughout Milwaukee County.
Qualifications:
- A Bachelor's Degree in Human Services or a related field, or an Associate's Degree with a minimum of 2 years of relevant experience is required.
- Experience working with children, families, or individuals with disabilities is essential.
- Knowledge of community resources to meet client needs is necessary.
- Ability to communicate and collaborate effectively with staff and clients in a positive manner.
- Strong time management skills and the capacity to handle multiple tasks simultaneously.
- A valid Wisconsin driver's license, reliable transportation, and auto insurance are required.
