We are seeking a compassionate and skilled Licensed Clinician to join our dynamic team at The Village for Families & Children. As a key member of our clinical staff, you will have the opportunity to make a profound impact on the lives of individuals and families in our community.
About the RoleThis is a half-time position in our Adult Outpatient Clinic, providing individual, group, and case management services to adults. You will also have the opportunity to work off-site with our Acute Community Support Team (ACST), providing higher-level care to individuals with severe and persistent mental/emotional illness.
Key Responsibilities- Conduct comprehensive clinical assessments and engagement
- Determine DSM V Diagnosis in consultation with supervisor, Medical Director, Psychiatrist, medical provider or designee as needed
- Develop treatment plans and contracts for services
- Implement treatment plans, modifying it as appropriate during the course of treatment
- Discharge planning, which may include linkages to community support services
- Provides case management services
- Completes clinical documentation required by agency and program policies, HIPAA regulations, and funding sources
- Maintains productivity targets determined by service
- Master's degree in social work, psychology, or other licensed counseling degree
- State of CT license required (i.e. LCSW, LPC, LMFT, Psychologist)
- Minimum of 2+ years of experience postgraduate in an outpatient setting (depending on program*)
- Bilingual/bicultural skills preferred
- Cultural competency to serve a diverse population
- Strong diagnostic, assessment and treatment skills for individual, family and group therapy
- Extensive experience working with culturally diverse populations, a wide age range, and a variety of clinical presentations
- Knowledge of behavioral health, child welfare systems, and community resources
- Good organizational and communication skills
- Ability to function autonomously in day-to-day responsibilities; yet seek consultation appropriately when needed
- Proficiency with word processing and general computer skills
