About the Role:

We are looking for an experienced LPN/LVN to join our team. As a Medical Professional, you will be responsible for providing individualized and group counseling, guidance, and health instruction to patients. You will also be responsible for maintaining accurate medical records, documenting patient interactions, and ensuring continuity of care.

Key Responsibilities:

  • Assess patient condition, recognizes adverse signs and symptoms, and reacts swiftly in emergency situations
  • Provide individualized and group counseling, guidance, and health instruction to patients
  • Maintain accurate medical records, document patient interactions, and ensure continuity of care
  • Work collaboratively with the PACT team to check-in/check-out patients and participate in staff meetings and huddles
  • Document all pertinent patient information in their medical records and VISTA to demonstrate quality care delivery and promote continuity of care

Requirements:

  • LPN from an accredited nursing school
  • Current and unrestricted nursing license in the state in which the LPN/LVN practices
  • Retain current Cardiopulmonary Resuscitation (CPR)/ Basic Life Support (BLS) from AHA or MTN
  • Minimum of two (2) years of current clinical experience working in a similar environment
