Medical Case Manager RN

Salary
The estimated salary for this position is $50 per hour, with the possibility of additional compensation elements including a sign-on bonus and discretionary awards. The total compensation package also includes a comprehensive benefits program, which includes medical, financial, and other benefits, such as 401(k) eligibility and paid time off benefits.
Job Description
As a Case Manager RN, you will be responsible for coordinating patient care and services, including those of the interdisciplinary team, social worker, chaplain, home health aide, volunteer, and physician. You will respond to necessary information to the Team Manager, monitor the re-certification/re-authorization process, participate in Interdisciplinary Team Meetings, and ensure/document appropriate use of medications, durable medical equipment, and supplies. Other key responsibilities include educating patients, families, and caregivers, providing skilled nursing as determined by the interdisciplinary plan of care, and participating in Quality Improvement assessments.
Key Responsibilities

Requirements
To be considered for this exciting opportunity, you must meet the following requirements:
- Current/valid Registered Nurse licensed in the State of California
- Minimum one year experience as a professional nurse within the last three years and holds a current Calif. RN license. Previous experience in hospice or home health preferred
- Knowledge of principles and practices of primary medical and nursing care
- BLS required. Demonstrates ability to use good listening and communication skills
- Able to multi-task effectively
- Capacity to relate and work as a member of an IDT
- Current California driver's license and proof of insurance
