Key Responsibilities
  • Collaborate with our interdepartmental clinical team and referral sources to ensure seamless care coordination.
  • Navigate the continuum of care with individuals seeking treatment, considering their holistic needs and ensuring linkage with all applicable services.
  • Process requests for school clearances and schedule assessments, utilizing your knowledge of mental health diagnostic criteria.
  • Triage incoming referrals and offer appropriate levels of care, utilizing your expertise in mental health and substance abuse.
  • Assist the Access Coordinator with the collection and maintenance of data systems, ensuring accurate and timely reporting.
  • Manage charting in our agency's electronic clinical record, adhering to standards set forth by our organization and funding sources.
  • Proficiency in Microsoft applications and general computer operations is required.
Requirements
  • Bachelor's degree in a Human Services related field.
  • Valid Driver's License.
