Key Responsibilities:
  • Collaborate with our interdepartmental clinical team and referral sources to ensure seamless care coordination.
  • Navigate the continuum of care with individuals seeking treatment, providing holistic support and linkage with applicable services.
  • Process requests for school clearances and schedule assessments, ensuring timely and efficient service delivery.
  • Triage incoming referrals and offer appropriate levels of care, utilizing your knowledge of mental health diagnostic criteria.
  • Assist the Access Coordinator with data collection and maintenance, as well as charting in our electronic clinical record.
  • Adhere to standards set forth by our agency and funding sources to ensure high-quality services.
  • Proficiency in Microsoft applications and general computer operations is required.
