Your expertise will help individuals navigate the treatment continuum, considering their unique needs and circumstances.
Key Responsibilities:- Collaborate with our interdepartmental clinical team and referral sources to ensure seamless care coordination.
- Navigate the continuum of care with individuals seeking treatment, providing holistic support and linkage with applicable services.
- Process requests for school clearances and schedule assessments, ensuring timely and efficient service delivery.
- Triage incoming referrals and offer appropriate levels of care, utilizing your knowledge of mental health diagnostic criteria.
- Assist the Access Coordinator with data collection and maintenance, as well as charting in our electronic clinical record.
- Adhere to standards set forth by our agency and funding sources to ensure high-quality services.
- Proficiency in Microsoft applications and general computer operations is required.
