Overview

We are seeking a skilled Nurse Triage Coordinator to join our team at Presbyterian Healthcare Services. As a key member of our Home Health and Hospice department, you will play a vital role in ensuring that patients receive the highest level of care and support.

About the Role

This is a dynamic opportunity for an experienced Registered Nurse (RN) with a passion for clinical triage and coordination. You will be responsible for reviewing inbound calls from patients and their families, assessing their needs, and providing guidance on appropriate care and services. Your expertise will help ensure that patients receive timely and effective treatment, reducing unnecessary hospitalizations and improving health outcomes.

About You
  • You hold a current RN license in New Mexico and have a minimum of 3 years of experience in a clinical setting, preferably in home health, hospice, or oncology care.
  • You possess excellent analytical and organizational skills, with the ability to interpret medical records and coordinate complex care plans.
  • You have a strong understanding of evidence-based guidelines and protocols, with the ability to apply this knowledge in fast-paced environments.
  • You are comfortable working varied shifts, including nights and weekends, and are willing to participate in occasional on-call assignments.
