Key Responsibilities

  • Care Coordination: Develop and implement comprehensive care plans that address patient needs, preferences, and goals.">
  • Patient Assessment: Conduct thorough assessments to identify patient needs, risks, and opportunities for improvement.">
  • Collaboration: Work closely with healthcare teams, including physicians, nurses, social workers, and other disciplines to ensure seamless care transitions and optimal patient outcomes.">
  • Documentation: Maintain accurate and timely documentation of patient care activities, including progress notes, treatment plans, and discharge summaries.">
  • Education: Provide education and support to patients and families on disease management, self-care, and health promotion.">
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care, reduce readmissions, and promote best practices.">

Requirements

  • Education: Bachelor's degree in Nursing or related field (Associate's degree may be considered with relevant experience)">
  • Experience: 1+ years of experience in nursing, preferably in a care coordination or case management role">
  • Licensure: Current RN licensure in New Mexico or eligibility for licensure">
  • Certifications: Basic Life Support (BLS) certification required; additional certifications may be preferred">

Salary Information

The estimated salary range for this position is $65,000 - $85,000 per year, depending on experience and qualifications.
