- Develop and implement individualized treatment plans, including medication management and lifestyle modifications.
- Collaborate with other healthcare professionals to ensure seamless care coordination and patient satisfaction.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- Master's degree from an accredited nurse practitioner or physician assistant program.
- Current unrestricted Texas license.
- Minimum 1-2 years of experience in family medicine or a related field.
- Current DEA certificate.
- Basic Life Support (BLS) certification.
- Experience working in a value-based care model.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
