HMH serves nearly 1,000 community members through our comprehensive program model, which understands and incorporates the strengths of individual cultures. Our team of over 40 full-time and 16 part-time staff speak multiple languages and dialects, enabling us to effectively communicate with our diverse community.

The Clinical Coordinator will be responsible for developing and implementing a project aimed at improving access to mental health care for A&YAs and their families. This involves conducting targeted outreach efforts and collaborating with community organizations to ensure timely and appropriate service referrals.

Key Responsibilities:

  • Develop and implement policies and procedures addressing A&YA needs
  • Collaborate with the Training Director to provide training and supervision on A&YA treatment modalities
  • Establish relationships with community stakeholders to facilitate referrals and outreach
  • Supervise Outreach & Community Health Workers in attending community events and meetings
  • Leverage internal resources by seeking referrals from HMH's Youth Development and Community Outreach departments
  • Develop and execute social media campaigns to reach potential clients and raise awareness about services
  • Provide education and training for parents/guardians of A&YAs and extended family members
  • Serve as initial contact for patients seeking behavioral health services
  • Coordinate intake assessment processes and connect patients with appropriate programs
