As a highly autonomous Independent Optometrist, you will be leading a team of eye care professionals at Sam's Club locations. This exciting opportunity allows you to establish your own practice, control your patient schedule, set your fee schedule, and keep 100% of exam and professional fees. While you'll have the freedom to choose your own days and hours of operation, you can also opt-in to equipment and instrumentation for a fair market value, as well as administrative and Opt-Tech support services (if state allows).



Key Responsibilities:


  • Provide high-quality eye and vision care services to patients
  • Establish and maintain a loyal patient base
  • Manage your own practice, including scheduling and patient care
  • Set your own fee schedule and keep 100% of exam and professional fees
  • Opt-in to equipment and instrumentation for a fair market value
  • Participate in ongoing education and training to stay up-to-date on the latest eye care techniques and technologies


Requirements:


  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school (if required by state)
  • Valid state ophthalmology license (if required by state)
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
