Job Summary

To facilitate effective discharge planning for patients with complex needs, ensuring timely and cost-efficient care.

The Complex Care Social Worker will utilize a multi-step escalation protocol to identify patients with barriers to discharge, collaborating with the interdisciplinary team to develop and implement a plan of care.

Responsibilities include:

  • Systematic gathering of pertinent information to inform discharge planning
  • Collaboration with the care team to develop and implement a plan of care
  • Monitoring of high-cost and difficult-to-place patients to identify early intervention opportunities
  • Leadership and creativity in developing programs and projects to enhance care management

Requirements:

  • Seasoned professional knowledge, equivalent to a Master's degree, with knowledge in more than one discipline
  • Current/Valid license in the State of Maryland; LCSW-C/LCPC required
  • Minimum of 3 years of experience; 5+ years of experience preferred
