Job Summary:

As a Social Worker at LifeBridge Health, you will play a vital role in providing direct services to patients and families to identify and resolve problems that impede discharge planning or the patient's use of healthcare resources.

This position involves contact with patients, families, physicians, nursing, ancillary staff, and internal/external resources. You will be responsible for assessing and documenting patient information, developing and implementing care plans, and collaborating with the multidisciplinary team to promote care coordination.

Key Responsibilities:

• Assess patient needs and develop individualized care plans

• Provide emotional, social, and financial support to patients and families

• Collaborate with healthcare teams to promote care coordination and patient outcomes

• Document patient information and care plans in a timely and accurate manner

Requirements:

• Master's Degree in Social Work/Counseling from an accredited institution

• Current licensure in the State of Maryland – LMSW/LGPC required; LCSW-C/LCPC preferred

• Minimum of 1 year of experience in a healthcare setting; hospital preferred
