We are seeking a Senior Social Services Professional to lead our Social Services Department at Avamere Rehabilitation at Park West in SW Seattle.

Job Description:

The primary purpose of this position is to plan, organize, develop and direct the overall operation of our facility's Social Services Department. We are looking for a compassionate and experienced professional to meet the emotionally and socially related needs of our residents.

  • Main Responsibilities:
  • Develop assessments and written plans of care to identify individual resident needs.
  • Ensure that all components of the care plan are being followed through regular review of MDS's and nurses' notes.
  • Communicate effectively with other departments and government agencies about patient conditions.
  • Participate in discharge planning and provide information to residents and families about available programs.
  • Maintain an atmosphere of warmth, patience, enthusiasm, and calmness within the department.

Requirements:

  • A Bachelor's Degree in a Human-Services related major (e.g., Social Work, Psychology, Sociology).
  • At least 2-5 years of experience in social services in a Skilled Nursing or similar healthcare setting.
  • Possession of certifications from organizations like the Academy of Certified Social Workers or membership in the National Association of Social Workers.
  • Knowledge of rules, regulations, and guidelines governing nursing care facilities.
  • Familiarity with Medicare, Medicaid, Kaiser, and private insurances.
  • Excellent oral and written communication skills.
  • Maintenance of confidentiality in accordance with HIPAA guidelines.

Estimated Salary: $67,000 - $82,000 per year, depending on experience, location, and qualifications.
