Job Summary

We are seeking a highly skilled Paramedic Transfer Facilitator to join our team at Yale New Haven Health. As a key member of our Patient Logistics team, you will play a critical role in facilitating the transfer of patients between our hospitals, ensuring seamless and efficient patient flow.

Key Responsibilities
  • Patient Throughput
    • Obtain accurate and pertinent medical history from referring physicians and institutions.
    • Direct referring physicians to the appropriate service line and work with Bed Associates and Clinical Bed Managers to ensure proper bed placement.
    • Document and report on patient activity timelines for efficient throughput.
  • Customer Service
    • Develop effective relationships with support staff and physicians to ensure patient flow and bed placement.
    • Communicate effectively and create an employee and physician experience that exemplifies caring, understanding, efficiency, and overall quality of service.
  • Census Verification
    • Properly document and follow policy and procedures related to processes developed.
    • Work with Bed Associates and Clinical Bed Managers to ensure proper placement at all hospitals.
  • Resource Management
    • Maintain a strong knowledge of EPIC and CRM applications.
    • Utilize strong multitasking and decision-making skills related to physician service lines and patient placement.
    • Communicate professionally and document accurate information.
  • Supports Organizational and Departmental Goals
    • Participate in workgroups with other divisions of the SBO.
    • Support team building and communication within the department.
Qualifications
  • Education
    • High school graduate or equivalent.
    • Completed EMT-Paramedic Program.
    • EMT-Paramedic Nationally Registered preferred.
  • Experience
    • Minimum of two (2) years of experience as a paramedic.
    • Experience in healthcare, including clinical practice, triaging, and transfer experience.
    • Demonstrated leadership experience with excellent judgment and prioritization skills.
  • Licenses and Certifications
    • EMT-P Certification required.
    • Continuing Education - Complete yearly CME's of 32 hours through approved credentialed provider.
  • Special Skills
    • Outstanding communication and facilitation skills.
    • Proficient in typing and data entry.
