Job Summary

We are seeking a skilled Hospice Registered Nurse / RN Case Manager to join our team. As a key member of our care team, you will play a vital role in enabling patients to spend quality time with their loved ones and doing the activities they enjoy.

Key Responsibilities
  • Build trusting, compassionate relationships with patients and their families
  • Empower caregivers by educating them on care for their loved one
  • Collaborate with the care team to provide comfort, peace, and dignity
Requirements
  • RN license in the state you work
  • Two years of experience as a registered nurse, hospice experience preferred
  • Current driver's license
  • Current CPR and BCLS certification
