Empath Health is in search of a Grief Support Specialist to enhance our dedicated team.

The Grief Support Specialist is responsible for providing emotional support and assessment for individuals impacted by the loss of a patient under the care of Empath Health. A personalized grief management plan is created by the Interdisciplinary Team (IDT) and is updated by the grief support specialist as necessary. Family members, friends, and caregivers of a Hospice patient may receive continued grief support for a minimum of 13 months following the patient's passing, as requested. The grief support specialist collaborates with the IDT to assist clients in navigating their grief journey and integrating their loss into daily life.

KEY RESPONSIBILITIES:

  1. Maintains a precise system for identifying cases requiring grief support services and a tracking mechanism for monitoring follow-up, ensuring compliance with grief support standards for timely client engagement.
  2. Conducts thorough grief assessments in partnership with the IDT and formulates a care plan for those family members or friends in need of support services.
  3. Evaluates client grief needs, coping levels, and implements clinical interventions as necessary during visits or calls.
  4. Facilitates appropriate referrals to internal resources and/or external community services.
  5. Educates team members and counselors on grief support standards and documentation practices, serving as a training resource within the organization on grief and loss topics.
  6. Manages clinical caseloads of team counselors who depart, as requested by the Director of Social Work.
  7. Provides ongoing support and education for grief volunteers, including training, orientation, assignment management, and documentation oversight. Collaborates with the designated grief volunteer coordinator as needed.
  8. Participates in meetings such as the Grief Care Committee, psychosocial staff meetings, and department-wide staff meetings.
  9. Pursues professional development through educational programs and maintains licensure requirements.
  10. Facilitates grief support groups at least twice a year for the community.
  11. Engages in after-hours on-call rotation as required.
  12. Contributes to quality improvement initiatives to uphold the highest standards of care, which may include committee participation, surveys, and chart reviews.
  13. Documents in the electronic medical record and adheres to guidelines for completing documentation according to established standards.
  14. Educates the community about Hospice Care philosophy and concepts through individual consultations or educational seminars.
  15. Performs additional duties as assigned by leadership.
QUALIFICATIONS:
  • Education and/or Experience: Master's degree (MSW, M.A. or MDiv) or equivalent; or four to ten years of relevant experience and/or training; or a combination of education and experience.
  • Graduate theological education from an accredited institution or accepted equivalency from a certifying body.
  • Minimum of 2 units of Clinical Pastoral Education; 4 units preferred.
  • Board Certification from an established accrediting body for professional chaplaincy such as APC, NACC, NAJC, and SCA preferred. Will consider Board Certified eligible candidates committed to completing the certification process within two years of employment.
  • Licensure:
    • LCSW, LMHC, LMFT, or licensed eligible and actively pursuing Florida licensure.
    • Valid Florida State Driver's License.
  • Continuing Education: As required for licensure.
  • Two (2) years of paid experience in a Social Work/Counseling role with a focus on grief support preferred.
  • Must possess excellent skills in assessment, communication, organizational ability, self-motivation, and technology.
  • For field-based roles, reliable transportation is required to perform tasks and responsibilities efficiently. Proof of valid automobile insurance is necessary.
