Job Summary

Highland Rivers Behavioral Health is seeking a skilled Behavioral Health Case Manager to provide direct care and support services to individuals impacted by mental illness, substance use disorders, and intellectual developmental disabilities.

Key Responsibilities
  • Assist individuals in identifying aftercare needs and developing a discharge plan through interviews with the individual, family, treatment team, and clinical staff.
  • Participate in treatment team meetings to assist with discharge planning needs.
  • Provide linkage to aftercare services by arranging appointments, transportation, and supports.
  • Monitor the individual's physical and emotional well-being and report unusual behavior or physical complaints to direct supervisor or Charge Nurse.
  • Assist in the admission and discharge process, including obtaining vital signs, safety search of individual and their personal belongings, and orienting new individuals to the unit.
  • Maintain clinical records and documentation according to agency policies.
  • Enhance professional growth and development through participation in educational programs, in-service meetings, and trainings.
  • Assist in prevention and managing crisis situations.
  • Respond appropriately to crisis situations that may involve physical intervention, including the use of seclusion/restraint.
  • Provide direct care for individuals as needed.
  • Transport individuals to approved designated locations at discharge.
Requirements
  • Minimum Qualifications: Bachelor's degree in a helping field with 2 years of direct experience with individuals in a psychiatric setting.
  • Other: Criminal background check clearance, official academic transcripts, valid driver's license/ID, drug screen, TB test, eligible 7-year MVR.
