We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) to join our team at Transition Home Healthcare. As a CNA-Home Health, you will play a vital role in providing exceptional care to our patients in the comfort of their own homes.
Responsibilities- Provide high-quality patient care in coordination with the assigned nursing care team.
- Maintain accurate and up-to-date patient records, including any changes in doctor's orders, diet, or personal living activities.
- Assist patients with daily living activities, such as feeding, bathing, and grooming.
- Deliver timely and accurate clinical notes to the case manager.
- Communicate effectively with healthcare providers and other care team members.
- CNA license in the state of Texas (required).
- One (1) year home health experience.
- Able to lift up to fifty (50) pounds.
- Able to work occasional weekends, holidays, and evenings.
- Strong communication and people skills.
- Experience with HCHB EMR (preferred but not required).
