Job Summary

We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) to join our team at Transition Home Healthcare. As a CNA-Home Health, you will play a vital role in providing exceptional care to our patients in the comfort of their own homes.

Responsibilities
  • Provide high-quality patient care in coordination with the assigned nursing care team.
  • Maintain accurate and up-to-date patient records, including any changes in doctor's orders, diet, or personal living activities.
  • Assist patients with daily living activities, such as feeding, bathing, and grooming.
  • Deliver timely and accurate clinical notes to the case manager.
  • Communicate effectively with healthcare providers and other care team members.
Qualifications
  • CNA license in the state of Texas (required).
  • One (1) year home health experience.
  • Able to lift up to fifty (50) pounds.
  • Able to work occasional weekends, holidays, and evenings.
  • Strong communication and people skills.
  • Experience with HCHB EMR (preferred but not required).
