We are seeking a compassionate and skilled Registered Nurse to join our team as a PRN (as-needed) hospice care provider. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals and families in their homes and communities.
Responsibilities- Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
- Provide expert nursing care, including medication management, wound care, and symptom management, to ensure optimal comfort and quality of life for patients.
- Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between healthcare settings.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
- Current RN licensure in the state of North Carolina.
- One year of hospice experience preferred, but not required.
- Excellent communication, assessment, and problem-solving skills.
- Ability to work independently and as part of a team.
- Strong commitment to delivering high-quality, patient-centered care.
