We are seeking a highly skilled Pharmacy Technician to join our team in a Medication Reconciliation role. As a key member of our pharmacy team, you will play a critical role in ensuring the accuracy of patient medication lists.
Key Responsibilities- Obtain accurate home medication lists from patients and caregivers, physician offices, pharmacies, and other relevant sources.
- Update medication lists in patient medical records to reflect accurate medications, dosages, frequencies, and indications.
- Collaborate with pharmacy, nursing, and medical staff to ensure accurate medication reconciliation.
- Support the Department's logistic, clinical, technical, and clerical functions in accordance with Department and institutional policies/procedures and State Board of Pharmacy regulations.
- High School graduate or equivalent.
- Minimum 1 year experience working in a pharmacy or administering medications in a healthcare setting.
- PTCB (Pharmacy Technician Certification Board) certification or Licensed Practical Nurse.
