About the Role

We are seeking a highly skilled and compassionate Patient Care Manager RN to join our team at LHC Group. As a key member of our hospice care team, you will play a vital role in ensuring the delivery of safe and efficient patient care that meets the highest standards of quality and compassion.

Key Responsibilities
  • Compliance and Regulatory Expertise: Ensure compliance with hospice policies and procedures, as well as local, state, and federal regulatory requirements governing the practice of the assigned team.
  • Quality and Safety: Actively promote and direct assigned team regarding quality of care and safety of patients and staff.
  • Problem Resolution and Complaint Investigation: Engage in thorough problem resolution and complaint investigation to ensure patient satisfaction and staff well-being.
  • Staff Education and Development: Provide in-services to staff to promote the health and safety of the patient, familiarize staff with regulatory issues and agency policy and procedures, and address any identified educational needs.
  • Leadership and Experience: Possess at least 4+ years full-time experience as an RN or equivalent, with a strong track record of leadership and patient care management.
Qualifications
  • License Requirements: Current RN licensure in the state of practice and one year of clinical experience.
  • Certifications: Current CPR Certification.
  • Transportation and Reliability: Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
