Job Title: Hospice Liaison Nurse

Novant Health is seeking a skilled Hospice Liaison Nurse to join our team. As a key member of our healthcare team, you will be responsible for assessing and admitting patients to Hospice care, facilitating discharge planning, and coordinating care with healthcare partners, facilities, patients, and their loved ones.

Responsibilities:
  • Assess patients' needs and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless transitions of care
  • Coordinate with patients, families, and healthcare providers to ensure timely and effective care
  • Monitor patients' progress and adjust care plans as needed
Requirements:
  • Current RN licensure in the state of employment
  • Minimum three years of experience in home care or hospice
  • Excellent nursing assessment skills and understanding of Hospice philosophy
  • Knowledge of Hospice services, Medicare/Medicaid rules, and nursing standards of practice
  • Strong communication and interpersonal skills
