In this role, you will work closely with patients and their families, providing personalized care and support throughout their transition from critical care to general medical-surgical units.
Key Responsibilities- Provide specialized nursing care to patients transitioning from intensive care to general medical-surgical units.
- Develop and implement individualized care plans to meet patients' unique needs.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Educate patients and families on post-intensive care recovery, medication management, and self-care techniques.
