Job Description Summary
We are seeking a highly skilled Care Coordinator to join our team at iCare Health Network. As an MDS Coordinator, you will play a crucial role in ensuring accurate reimbursement for services by overseeing and guiding the process of Medicare/Medicaid case-mix document tracking and care assessments. This role requires excellent communication and organizational skills, as well as the ability to work effectively with various departments, including Nursing, Dietary, Social Services, and Rehabilitation.

About the Job Requirements: To be successful in this role, you must have a strong understanding of the MDS process and be able to coordinate resident care assessments, planning, implementation, and evaluation. You should also be able to facilitate care conferences with the interdisciplinary team, complete and review Minimum Data Sets (MDS), educate and train nursing staff and other departments on assessment roles, planning, implementation, and MDS completion, and ensure all current MDS/PPS updates are implemented and staff is in-serviced.

Key Skills and Qualifications:
  • Active nursing license in the State of Connecticut, required.
  • Knowledge of Quality Assurance/Improvement processes, required.
  • Bachelor's or Associate degree in a healthcare-related field, preferred.
